Support Qualified External References in All Forms

External references which are artifact qualified are supported for a element names e.g. as for primitive type external references as http://www.omg.org/spec/UML/20110701/UML.xmi#String. Add artifact and package qualified mappings, where they don't already exist, for all basic elements by both name and UUID. Inconsistencies exist in the mapping "layer" where some element types are being mapped as artifact URN qualified and not artifact URI qualified. E.g. see InMemoryMapping.mapDataType(...)
