Redesign user settings UI

The current user settings UI lists each fuzzer as a checkbox. It would be nice to have a more responsive design that allows us to select fuzzers from a drop-down and add these to the current subscribed crash list.

The same should be applied to notifications. Notifications could also be expanded to allow setting more explicit notification matches.
