To make it real-time, the main concept points are as follows

Use mongoDB indices to avoid duplicate insertion due to concurrency
Add support to generate sort order IDs for multiple fields after contacting those fields to support sorting on multiple fields at once
Use n-ary search to make it efficient (Already done once)
