Provide ability to set default options for plugin

We are a University and we have had a few occurrences of questionnaires being used to collect (unfortuntely) sensitive information in the belief that responses would only be visible to non-student roles.

This is a feature request to add additional configuration items to the admin screen of the plugin to be able to set default options such as "Students can view ALL responses" for any new instance of a questionnaire on a course.
