@mchurchward We are running the following versions of Moodle and Questionnaire:

A faculty member has found multiple questionnaires in their course that have displayed the following issue:

  1. Not editable. As you can see in the image below, some of the options related to editing a questionnaire are not available from the edit menu.

  2. When I preview the questionnaire, I see a different name than I am expecting.

I have attempted to duplicate the questionnaire, but the duplicate is broken as well.
This probably stems from a bug in a previous version of questionnaire and it has been passed down through restore/import until the questionnaire needed to be edited.

I noticed this if the questionnaire was created in another course and the type was set to public (in advanced). Even if I have rights to edit each course (e.g. I'm the site administrator or a manager etc) I have to open the course that the survey was created within in order to get the modify options. It's not necessarily obvious which course the questionnaire was created in.
