This is a forwarding of a request/discussion made in the discord regarding a new feedback form for the upcoming drug combination chart.

What is your name?
What suggestions do you have for the combination chart?
(optional) Contact information

No need to force a login for anything, maybe some captcha if there's a concern of spam, and we could have it automatically dump into a spreadsheet for easy review.

It is on the roadmap, I love this idea.

As tripsit thrives on public research, we should by default definitely have a public feedback option.

Agreed. Especially as an easy QR code on the chart would make it so easy to contribute.

We do have ways to send feedback, but something streamlined will help capture a lot of the feedback often solely posted on reddit/bl/etc and then unfortauntly not seen by us.
