This PR fixes a bug in the GEPA optimizer where it would ignore the user-provided instruction and fall back to a default. The fix involves adding a getInstruction method to the AxGen and AxPromptTemplate classes and updating the optimizer to use it. A new test is included to verify the fix.

The GEPA optimizer was failing to use the instruction provided to a program, instead falling back to a default instruction. This was caused by an incorrect attempt to access a non-existent `instruction` property on the `AxSignature` object.

This commit resolves the issue by:
1. Adding a `getInstruction()` method to the `AxPromptTemplate` and `AxGen` classes, providing a proper way to access the program's instruction.
2. Updating the `getBaseInstruction` method in the `AxGEPA` optimizer to use this new method.
3. Removing a faulty `try-catch` block that was suppressing the original error.
4. Adding a new test file (`gepa.test.ts`) to verify that the optimizer correctly uses the program's instruction.
