If users are going through this, it may be ugly or confusing to have notes for every single question. At same time, if there is only a single notes at end of section, one-off users may not use it.

If in group setting, either A or B could be used.

= Use case =
When stepping through the questions with a group of people, there are often quite a bit of discussion. Some of that discussion may be worth noting. Ex: overall, the company scores low here, but the OA group has pocket of high open source use
