Several browser extension hiccups

I used this page for testing -- #4

Please find below 3 ways of adding task notes, all inconsistent, all with some hiccups (opinionated?).

Context menu + "Add page to TaskNotes"

See the added review tag. I do not need it and it is not added by the other two methods below.
What should I do in order to avoid adding this tag?

Toolbar icon + "Create Task" button

See the source URL added twice. The second is very much redundant.
What should I do in order to avoid adding the second reference?

See the added text "Added from browser extension". It is not really useful.
Is it possible to avoid adding the text "Added from browser extension"?

Added from browser extension

Toolbar icon / Dialog / No buttons to click right away

This is what I get. Notice no buttons available for creating a task, I have to scroll to find one of the two available.
At the same time not so useful "Test Connection" button consumes precious vertical space.
