Updates to the controller header

This ticket calls for some decently sized structural changes to the way we render the header and controller details (i.e. Username, address, network)

See phantom for some examples of this behavior. I pulled a lot from their designs.

in short... we no longer have a title block for the Inventory, Activity, and Achievements pages. Nav is moving to the footer (#1171 ) and the network and Controller address are moving into a menu that can be seen when clicking on a username button in the header.
