Display token counts for files and directories to help users easily identify and exclude large items

Problem / motivation

Currently, it's difficult to identify which specific files or directories are contributing the most to the total token count. This makes it challenging to use the "Exclude" feature effectively, especially in large repositories. Without this visibility, I might unknowingly ingest very large, unnecessary files, which increases processing time and potential costs.
