How should we be installing this extension? Was it removed from the marketplace? And if so, why?

Yes I was forced to remove it from the marketplace due to license issues. For more information see here: microsoft/vscode-discussions#1 (comment)

You can download the .vsix file from the release section and run "Install from VSIX" in VSCode.

It forks from this repo. Was curious if it adds anything meaningful on top of your fork but from skimming, doesn't seem to be the case.
