Error message: found a token but failed getting the clear version

We sometimes get this error message from the parser:

What does it mean? Is it a serious issue or can we ignore it? If it's not serious, maybe it would work better as an Info or Debug log. If it is, maybe it could be clarified so we know how to act on it.
