Complete distributed key generation

Current implementation only covers phase one (out of five) of the distributed key generation protocol. Concurrent phases should be completed:

• Phase 2 (started in [WIP] Committee changes---Round 2 #573 )
• Phase 3
• Phase 4
• Phase 5
