Verify record→paste end-to-end on macOS

Manual smoke test (needs mic + interactive permission grants):

• v2t — grant Accessibility + Input Monitoring when prompted
• Hold Right ⌘, speak, release → cleaned text pastes at cursor, clipboard restored
• v2t --casual, v2t --no-cleanup, v2t --backend whisper
• brew install swiftbar + drop swiftbar/v2t.5s.sh → menu shows status, Start/Stop, open folders
• Confirm ~/.v2t/history/transcriptions.jsonl is written

Resolved/superseded by PR #2. The native menu replaced the SwiftBar/Input Monitoring flow; record→paste, clipboard restoration, permissions, private JSONL history, and menu/engine lifecycle were validated on macOS. Optional modes are covered mechanically; cross-model benchmarking remains tracked separately in #3.
