Addition of two new 2D diagnostics: cloudsat total cloud fraction, and cloudsat 
total cloud fraction neglecting the bottom km of atmosphere.
Details of code changes in e-mail (21/10/2011), and in attached file with 
comments modified by ZYY.
Missing updates to mip tables and I/O routines.
