Track pending DAP output writes, wait to signal finish until every queued nio.run task flushes.

DISCLAIMER: PROBLEM WAS DISCOVERED AND FIXED BY LLM AGENT
After fixing #556 I asked the agent to find and fix similar race conditions in the codebase and it produced this patch.
The control flow is more complex than in #556 so someone experienced need to review it but it all make sense on the first glance.
