I have CCv3 deployed on my root level projects folder

EG /projects/
were all the projects are in subfolders.

When I say "done for today" CC does not seem to always run the create_handoff skill, instead choosing to create a memory.md which does not get fed into the database.

Is there any downside for me either running the create_handoff manually, or is a hook missing? TLDR appears to fire most of the time, so some/most of the hooks are running.

If I re-run the setup wizard, to check settings, I am concerned it will overwrite the memory.

2. Fallback — if the embedding model times out, I save to file-based memory instead (~/.claude/projects/.../memory/).

The point is that you shouldn't have to ask separately — ending the session is the trigger to persist what we learned so future sessions can recall it.

A couple of related things that may also kick in depending on the work:

• If we made code changes, the deployment workflow still applies (version bump in config/app.php, CHANGES.md entry, and a "Deploy now?" prompt).
• A create_handoff skill is available if you want a formal handoff doc for the next session — but that's optional and only if you ask.

This is just an explanation — you haven't ended the session, so I haven't stored anything yet. Say "done for today" when you actually want me to wrap up.

The reason you keep seeing the "🎯 SKILL ACTIVATION CHECK → create_handoff (REQUIRED)" banner is a hook misfire — the UserPromptSubmit hook pattern-matches session-ending language ("done for today") and wrongly maps it to create_handoff instead of the store-learnings behavior. It's been
