Create a view to handle the add/edit form for assessments.

Template should include:

Heading/Title/Assessment Name (text box)
Description (textarea)
Required (yes/no)
If assessment is pre or post test:
- passing score (text box)
- max attempts (text box)
ability to create from a template
ability to save assessment as a template
