Add 'surprise/warning' field to the recommendation form

After some recent College of Heralds discussions, the recommendation form should have radio buttons, or something similar, to allow the recommender to indicate whether the recipient would most likely want to be surprised in court or given some kind of warning/summons. Some people prefer to be surprised, some find surprise to be extremely stressful.
