AddData.js 500 error CreateNewSignal

Deploying ATSPM 4.2. We have an issue with the Create New Signal link. It seems there may be an issue with a reference in a script; the user isn't directed to another page or pop-up with a form to enter the information and a 500 error results. The link address is javascript:CreateNewSignal().

I am having similar issue. Nothing happens after I click Create New Signal.
