• Prefer exact provider-returned cost when available.
• Support provider model-catalog pricing metadata.
• Allow custom providers to configure input, output, cache-read, and cache-write rates.
• Do not silently treat non-USD prices as USD.
• Clearly mark estimates versus provider-reported costs.
• Add tests for unknown models, dated model aliases, caching, and per-session totals.
