PIV loop review for ROY-23

Result: PASS

Reviewed HEAD (241e691) changes in src/workflow.ts and tests/workflow.test.ts for regressions and test coverage; the new token-usage formatting and propagation paths are consistent with existing workflow behavior, include sensible fallbacks (unknown) for missing usage data, and are covered by targeted unit tests for full, partial, and absent usage payloads. Quality gates all passed on May 6, 2026: bun run check, bun run typecheck, and bun test (45 passing, 0 failing).
