PIV loop review for ROY-24

Result: FAIL

Static review found two workflow regressions despite all local gates passing (bun run check, bun run typecheck, bun test): explicit issue runs can be skipped when the issue is not in the assigned state and there is no prior local run-state, and all-project polling behavior is derived only from the first project’s polling config, which can cause premature stop or unintended continued polling when projects have different polling settings.

Bugs were detected and converted to GitHub issues.
