Related to #2

I'll try to keep PRs much smaller than this in the future so they're easier to review.

Includes:

• Some of the requirements discussed today (not all of them, need to upload that list)
  • NOTE: Some of the requirements have FIXMEs, they are set to active:false
  • When a PR that sets them to true comes along we can review the wording then, these are effectively in draft
• A README on how to use doorstop in context
• A couple of python scripts and a bash script for formatting and running specific hooks

Learned to make custom fields so we get something like this in the HTML:

The custom hook in doorstop_hooks.py makes the custom rationale field required.

Still working out how to get the default values to work for custom attributes, not appearing to work so far...

Actually did some major refactoring since, going to close this without merging and open two new PRs:

• one for just the infrastructure (Doorstop Infrastructure #6 )
• and one for the high level requirement files and some initial children.
