This PR replace the higher/big/ng references with a single universe-polymorphic (ghost) (PCM) reference type. The allocation function takes an extra
{| small_type u#a |}argument restricting the universe level.Depends on FStarLang/FStar#3699 (Otherwise it's way too easy to mess up universes.)
