I've not seen the raw data from previous years surveys. Do people put in some sort of contact information when filling out the survey like an email or phone number? Trying to make sure we don't ask this question and then have no way for the SC to reach out to people.

Nothing stops you from having an optional "email" field if you need. :)

Not by default, but yes we could add a field conditional on that question.
