Move MCP server setup logic into _create_mcp_server() function that both CLI (main) and Lambda (create_mcp_http_app) can use. Lambda handler now imports from the main module instead of duplicating configuration.
Add back detailed comment explaining why the InvalidStateReference schema needs manual patching to break circular references for FastMCP.
