The reader of the README doesn't care that hearsay was built in
phases or that the auth model pivoted between v0.2 and v0.3. They
care what hearsay does today.
Replaces every 'Phase 1 / Phase 2 / Phase-3' mention and every
'v0.2 / v0.3' version reference with prose that just describes the
current behavior. Two top-level capabilities ('past sessions' and
'live queries') stay in the lead, but lose the phase numbering.
Tools-table prefix changes from '**Phase-2.**' to
'**Requires `--enable-agent`.**' so the reader sees the actual gate
instead of an internal-codebase label. Design notes section
headers drop the (Phase 1 / Phase 2) suffixes. The 'Removed in
v0.3' sentence becomes 'Not supported' so a reader who has never
seen a previous version doesn't have to wonder what's going on.
