Summary

add PDF ingestion utility with text extraction fallback to OCR via gpt-4o-mini
trigger normalized text persistence and batched question generation during upload
include submission summaries in chat context, expose questions endpoint, and document manual test flow

Testing

not run (npm install for new native dependencies is blocked by the execution environment proxy)
