docs: mark context-budget campaign practically complete - #73

Update public-facing documentation to reflect the completed context-budget/reference-extraction campaign. README and CONTEXT_BUDGET_PLAN now describe the compact always-loaded kernel, lazy-loaded references, validation posture, and practical completion status. No runtime behavior changes. Replace literal `<scrubbed>@gmail.com` references in the 2026-05-05 history-rewrite decision log with neutral phrasing (`<redacted-email>`, "personal email address", "personal email pattern"). The incident record and the lesson are preserved; only the grep-noisy email-domain literals are removed. No safety automation or runtime behavior changes. Vendored upstream files untouched.

Update README.md and docs/CONTEXT_BUDGET_PLAN.md to reflect the completed context-budget / reference-extraction campaign.
Adds final campaign status: practically complete at ~95% by impact, with end-state evidence, remaining large-file classification, and clear re-open criteria.
Also neutralizes grep-noisy historical email-domain wording in the PII/history-rewrite decision log while preserving the incident record and lesson. Remaining broad-grep hits are documented false positives or load-bearing literals: MCP/tool names, REST route examples, safety-check patterns, public support addresses, and vendored placeholders.
Validation passes:
After this PR merges, declare the context-budget / reference-extraction campaign complete at ~95% by impact / practically complete.
