I'd like to see this PR go through. A few questions though. Does a todo file extension need to be defined? Not sure if that can be done via the extension.

 "files.associations": {
    "*.todo": "todo"
  }

Also most examples wrap the LangId in single quotes
editorTextFocus && !editorReadonly && editorLangId == 'todo'
